Use a suitable device and check every section
The university recommends a desktop or laptop for the form and asks students to complete every section.
Choose a time when you can work through the paperwork without being rushed. Before finishing, review the fields and contact information. If a question does not fit your situation or a required field will not accept an answer, ask the office how to handle it. Describe a technical problem using the section name and error message rather than sending a copy of your entire intake in an ordinary support email.

Track submission separately from the scheduling response
Concordia says a counselor reviews the submitted forms before contacting the student to schedule.
Note when you completed the intake and any receipt or next-step message displayed. Check the contact channel you supplied for a response. When the counselor contacts you, confirm the date, format and any remaining paperwork. If you only have proof that a form was submitted, keep the appointment task open. A submission record and a visit confirmation answer different questions.

Follow the two-business-day contact instruction
The New Clients page tells students who have not heard from counseling within two business days to email the office.
Use that published contact and include the date you submitted the intake and the practical issue: you are checking whether it was received and what happens next. Ask before starting a duplicate submission if the original appears missing. If the office gives you revised instructions, record them with the original date so you can follow the process consistently. The follow-up instruction identifies when to contact the office; it is not a promise that your appointment itself occurs within that period.

Considering therapy or medication management at Lyte
Lyte provides therapy, psychiatric evaluation and medication management. Ask which service and clinician fit your needs. A medication evaluation does not guarantee a prescription or continuation of a particular medication.
Lyte Psychiatry offers telehealth visits to patients who are physically located in Texas at the time of the visit. In-person appointments take place at Lyte’s Pantego clinic; this campus guide does not describe a Lyte office at the institution.
Coverage and costs depend on your specific plan, clinician, visit type and deductible. Confirm network status and expected costs before booking. A student plan, parent’s plan or campus referral does not by itself establish Lyte coverage.
Request a Lyte appointmentConfirm appointment format, availability and costs before booking. For questions, call 469-733-0848. Clinic hours are Monday–Friday, 9:00 AM–5:00 PM Central Time. Weekend appointments are limited and offered case by case.
Send medical records and insurance details only through the practice’s approved secure intake process.
Questions before you arrange care
Do I need separate Concordia intake forms for coaching and counseling?
No. Concordia’s New Clients page says the paperwork is shared. Ask the counselor to discuss the service choice if you are uncertain.

What if I have no response after submitting the intake?
Follow Concordia’s instruction to email counseling if you have not heard back within two business days. Include the submission date and ask about receipt and the next scheduling step.
