Forms and paperwork
Every form is completed and signed inside the client portal — that is the secure channel, and it means nothing has to be printed, scanned or posted.
What you will be asked to complete
New patient intake
Before your first visitSent to you through the portal after you book, and due before the first appointment. Demographics, history, current medications, pharmacy, insurance and consent to treat. It takes about 15 minutes and it saves the same 15 minutes out of your appointment.
Consent to treat and financial agreement
Before your first visitIncluded in intake. Covers what treatment involves, the limits of confidentiality, and your financial responsibility including the cancellation policy.
Release of information (ROI)
Any time you want us to coordinateNeeded before we send records or speak to anyone outside the practice — your primary care physician, a therapist elsewhere, a school, a family member, an attorney. Each release names one recipient and what may be shared, and you can revoke it in writing at any time.
Records request
On requestRequest your own records through the portal or in writing. Texas law gives a provider 15 days to furnish records after a written request; we aim to be faster. A reasonable copying fee may apply for large records.
Telehealth consent

Notice of Privacy Practices
Always availableHow your health information is used and disclosed, and your rights over it under HIPAA. Provided at intake and available on the site at any time.
Letters and clinical documentation
Ask at an appointment rather than by message where you can — most letters need a conversation about what the recipient actually requires. Allow five business days, and longer for forms that run to several pages.
What we can provide
- A letter confirming you are a patient, your diagnosis, and your treatment dates
- Documentation of diagnosis and functional impact for a school or employer disability office
- Standard FMLA and short-term disability paperwork, completed from what is in your record
- A letter supporting a workplace or academic accommodation request
- A letter for an emotional support animal, only where it is clinically justified by your record
What we cannot
- A back-dated note for an absence that happened before you were our patient
- A note confirming something we have no clinical basis to confirm
- Court-ordered evaluations, custody evaluations, or forensic assessments
- Disability determinations for Social Security — those are made by SSA, though we supply records
- Documentation for a condition we do not treat, including ADHD
A letter records what is in your chart. If something is not documented because it has not been assessed, the honest answer is that we cannot write it — but we can usually assess it at your next visit and write it afterwards.
