Choose the office contact or Care Referral Form
The appointments page describes requesting a visit through the counselor’s office, by contact with the office or by the Care Referral Form sent to the counselor. It identifies the office in the lower level of the James W. Dillard Library.
Use the current page for the contact details and say whether you are starting a request or following up on one already sent. Before making a trip, verify the office location and a suitable time to speak with staff. Ask how you will receive the appointment details. Sending a form gives the office a request to handle; it should not be treated as a confirmed time by itself.

Answer the form’s purpose and awareness questions accurately
The form asks whether the referral is for yourself or someone else and whether the person is aware of it. It also distinguishes a request for information, an appointment or someone to make contact.
Read those choices carefully and describe the action actually requested. If you are helping another person, do not claim they asked for an appointment when you are only seeking information about available support. Provide factual details relevant to the request and follow the form’s instructions. If you are unsure which choice fits, ask the office before interpreting an outreach request as an appointment booking.
State whether you prefer an in-person or online visit
The appointments page asks students to indicate their in-person or online preference. The personal-counseling page also describes both formats.
When making the request, explain the format you can use and any scheduling constraint. If online, ask about the connection instructions and where you may be located for the visit. If in person, confirm the office attached to the appointment. A preference is information for the scheduler, so wait for the actual arrangement before planning around it. Keep any format change with the new date and time.

Use the published contact when canceling or rescheduling
Frank Phillips asks students to contact the office by eight in the morning on the appointment day for cancellation or rescheduling. Its instructions allow a voicemail outside business hours.
If you know you cannot attend, contact the office promptly and identify the appointment affected. Ask whether the original visit has been canceled and what remains necessary to arrange another time. Retain the date and channel of your message, then update your calendar once the office confirms the change. If the published deadline has already passed, still contact staff to explain the situation and ask how to proceed.

Considering therapy or medication management at Lyte
Lyte provides therapy, psychiatric evaluation and medication management. Ask which service and clinician fit your needs. A medication evaluation does not guarantee a prescription or continuation of a particular medication.
Lyte Psychiatry offers telehealth visits to patients who are physically located in Texas at the time of the visit. In-person appointments take place at Lyte’s Pantego clinic; this campus guide does not describe a Lyte office at the institution.
Coverage and costs depend on your specific plan, clinician, visit type and deductible. Confirm network status and expected costs before booking. A student plan, parent’s plan or campus referral does not by itself establish Lyte coverage.
Request a Lyte appointmentConfirm appointment format, availability and costs before booking. For questions, call 469-733-0848. Clinic hours are Monday–Friday, 9:00 AM–5:00 PM Central Time. Weekend appointments are limited and offered case by case.
Send medical records and insurance details only through the practice’s approved secure intake process.

How does Frank Phillips ask students to cancel or reschedule?
